Our Founding
Founded in 2026 with a mission to eliminate the aged care bed-blocking crisis in Australia, Discharge2Care was born from first-hand observation of the devastating impact of hospital discharge delays on patients, families, and the healthcare system.
In NSW alone, over 1,100 patients occupy hospital beds while waiting for aged care placement — costing the system an estimated $438 million annually. Patients like James Brown have died from hospital-acquired infections while waiting. NSW Health Minister Ryan Park has publicly called the situation “beyond frustrating.”
Discharge2Care was created to solve this. Our platform compresses the average placement cycle from 48 days to under 10 days using a unified digital workflow that replaces phone calls, fax machines, and manual approvals with verifiable, auditable, and tamper-proof coordination.
