This policy explains how we collect, use, disclose, and safeguard your information when you use the Discharge2Care platform.

- Account information: name, email address, role, organisation affiliation.
- Clinical data: de-identified patient placement records, ACAT assessment references, discharge milestones (processed on behalf of healthcare providers).
- Technical data: browser type, IP address, access logs, session identifiers.
- Blockchain records: transaction hashes, escrow states, and credential proofs recorded on the XRP Ledger (public, pseudonymous, and non-reversible).
3. How We Use Your Information
- To operate and maintain the Discharge2Care platform.
- To facilitate aged care placement coordination between authorised parties.
- To generate auditable records of placement milestones and financial escrows.
- To comply with legal and regulatory obligations, including NSW Health reporting requirements.
- To improve platform performance and user experience.
4. Data Security
We implement industry-standard technical and organisational measures to protect your information, including encryption in transit (TLS 1.3), role-based access controls, cryptographic credential verification, and immutable audit trails on the XRP Ledger. Patient identifiable information (PII) is never stored on-chain — only cryptographic hashes and pseudonymous identifiers.
5. Disclosure of Information
We do not sell your personal information. We may share information with authorised healthcare providers, ACAT assessors, and aged care facilities solely for the purpose of facilitating patient placement. We may also disclose information where required by law or to protect the safety of patients.
